Many patients prefer in-home nursing to the hospital, and our Daytona Beach home health care program is designed to bring our expertise and quality services to the comfort and familiarity of our patients’ homes. The AdventHealth Home Health program, available in both Volusia and Flagler counties, is licensed by the Agency for Health Care Administration (license #HHA215580961), certified by Medicare, and accredited by The Joint Commission.

AdventHealth Home Health Care encompasses a wide range of health and social services. Our home health care team is available seven days a week for recovering, disabled, chronically or terminally ill patients in need of medical, nursing, social or therapeutic treatment and/or assistance with the essential activities of daily living.

Individualized Program to Meet Your Specific Needs

Assisting patients with individualized medical care at home is our specialty. Home care may begin immediately after hospitalization (though prior hospitalization is not required) or as a direct referral from your physician, though hospitalization is not required to qualify for our home health services. With the guidance of your physician and coordination by licensed medical professionals, we provide the following specialized services:

Qualifying for Home Health Care

AdventHealth Home Health accepts most insurance plans, including Medicare. Our services are available to anyone who:

  • Is under the care of a physician (only a physician can order home health services for their patient)
  • Lives in Flagler or East Volusia County
  • Is considered “homebound” and requires the skills of a registered nurse, physical therapist, and/or speech language pathologist, during the course of treatment

To qualify for benefits under the Medicare program, you must be considered “homebound”. Medicare considers a person to be “homebound” if:

  • You require considerable and taxing effort to leave home.
  • Absences from the home are infrequent and of short duration.
  • If your physician has advised that it is unsafe for you to leave home due to your medical or psychiatric condition.

Examples of homebound status include:

  • A patient who drives and can leave home without assistance is not considered homebound. Circumstances that qualify a person as homebound include:
  • Walks with a walker, has difficulty going up and down steps to leave home; must have help to get in and out of car
  • Can walk without direct assistance, but for short distances due to shortness of breath or other physical impairment
  • Limited mobility caused by extreme weakness or severe pain
  • Confused and unable to leave home unassisted
  • Depression or withdrawal; refusal to leave home

Examples of allowable absences from the home include:

  • Church attendance
  • Beauty salon services
  • Other non-medical reasons if absences are infrequent, of short duration and require a considerable and taxing effort
  • Visit to hospital, doctor’s office or healthcare facility
